Office Policies
What patients should know
- Copayments, deductibles, coinsurance, and balances are due according to the practice's financial policy.

Record requests
Medical-record requests and releases must be handled through an approved secure process. A signed release of information is required before records can be shared. Records cannot be uploaded, requested, or delivered through this public website or by ordinary email.
