This website is designed to collect as little information as possible. The general administrative contact form asks only for your first name, last name, telephone number, email address, preferred contact method, a general reason for contact, an optional short administrative question, and your consent to be contacted.

Submissions are not stored on this website. The form opens a message in your own email application addressed to the practice's administrative inbox.

Information you should not submit

Do not enter symptoms, diagnoses, medication information, screening answers, treatment details, Social Security numbers, medical-record numbers, insurance identification numbers, or other private medical information into any form on this website. Ordinary email and website forms are not secure and are not HIPAA-compliant channels. For patient-specific concerns, call the office at 757-863-5843.

Clinical information and medical records

Appointment requests, patient registration, clinical questionnaires, and intake forms are submitted through this website's secure forms system and sent directly to the office. Medical records cannot be uploaded through this public website.

Payments

Payments are processed through a checkout page hosted by Stripe. Card details are entered on Stripe's page, not on this website, and this website does not store card numbers. Diagnoses, medications, appointment types, treatment names, and other protected health information are not transmitted to the payment processor. See the payment page for details.
